An electronic recommendation form will be sent via email to the teacher and counselor/coach/mentor you include in your application.
References have until February 24, 2023 to complete the recommendation form.
Note: Please make sure the email address you provide for each references is correct. Once you submit, you will NOT be able to make any changes to the form.
Your application will not be considered if the forms are not completed by the deadline. You will receive a confirmatin email per completed reference once it has been received. Please check individually with your teachers and outside referrences to ensure they have received the email request. Some school districts have strict firewalls that prevent the arrival of outside emails or they might end up in SPAM/JUNK filters.

Volunteer Commitment

The volunteer placement process includes several steps and trips to the hospital before you can begin volunteering (this may total as many as four trips to the hospital). The steps you must complete to become a Volunteer are: 1. Submit an Application 2. Schedule a virtual Interview (a parent/guardian must be present) with the Volunteer Department 3. Complete a Health Clearance Examination through CHLA Employee Health Services Department (a parent/guardian must be present) 4. Complete Competency Tests/Documents 5. Attend a 6-hour Volunteer / Patient Care Orientation on June 11, 2023 6. Receive a Start Date Are you willing to make these trips and the required steps to complete the volunteer placement process?

CHLA's High School Summer Volunteer program is very structured. The High School Summer volunteer commitment is for 105 service hours. Do you have time in your life for this regular commitment?
